Scrub Purchase Order Template: 12 Fields for Accurate Orders

A scrub purchase order template connects an approved buying decision with the quantities, product references and commercial details the supplier must acknowledge. It should let a merchandiser, production planner and receiving team identify the same order without reconstructing instructions from scattered messages. For medical uniforms, the most important distinction is often the unit: a scrub set, a top and a pair of trousers are not interchangeable purchasing quantities. This guide provides twelve fields to organise, an illustrative size breakdown and a reconciliation method. Adapt the structure to your purchasing system and agreed contract; it is an operational worksheet, not a substitute for reviewed contractual terms.

1. Give the scrub purchase order template a unique identity

Start with a purchase order number that remains unique in your buying system. Add the issuing organisation, supplier, issue date and responsible buyer. Record the relevant quotation and agreement references so the recipient can connect the PO with the offer you accepted. Avoid using a product name as the only identifier because the same style may be ordered several times under different conditions.

2. Identify the product with a style code and description

Give every ordered product a stable style code and a short description. For a coordinated set, identify the top and trouser components and the approved combination. Describe separate jackets, lab coats or caps on separate lines when their prices, quantities or delivery arrangements differ. A supplier should be able to distinguish two similar products without relying on a thumbnail or a colour name.

Link the description to the relevant scrub set specification and any separately ordered scrub jackets. In a scrub purchase order template, a descriptive name helps people read the document, while the code provides continuity across revisions, cartons, invoices and your stock system.

3. State the unit of measure before entering quantities

Define the unit of measure beside each order line. If one set contains one top and one trouser, state that relationship explicitly. An order for 1,200 such sets represents 2,400 individual garments, before any separately ordered products are added. This arithmetic does not establish a factory’s output rate or its available capacity; it simply prevents the two parties from counting the same order differently.

4. Include a complete colour size breakdown

Prepare a colour size breakdown for each style rather than giving only a grand total. Record the approved colour reference and the size labels exactly as they will be used for the order. Where tops and trousers can be ordered in different sizes, identify the permitted combinations instead of assuming that every set contains matching labels. Keep size quantities tied to the correct product code.

5. Reconcile the order quantity with a worked example

The example below is illustrative and contains one style in one colour. It is not a recommended size ratio, a customer order or confirmation of stock. Its purpose is to show how a buyer can connect size quantities with a set total and then with the number of individual garments. Your actual ratio should follow your customer or staff requirements and the agreed ordering conditions.

In this example, 60 plus 240 plus 360 plus 300 plus 180 plus 60 equals 1,200 sets. With one top and one trouser in each set, the total is 2,400 garments. Keep this quantity reconciliation separate from carton calculations until the packing method is confirmed. A different pack size can change the carton count without changing the number of ordered sets.

6. Record unit price, currency and line totals clearly

Put the agreed unit price next to the same unit used for the quantity. Calculate the line total as the ordered units multiplied by that unit price, and identify the currency. Show separately agreed charges and exclusions rather than concealing them inside an unexplained total. If prices differ by product or another agreed factor, use separate lines that make the difference visible.

Do not silently replace a quoted quantity, delivery basis or packaging scope while retaining the old price and assuming the offer still applies.

7. Attach the current technical and sample references

Identify the approved tech pack by filename, revision and date. Include the measurement chart, fabric reference, colour standard, construction details and accepted sample identifier as relevant to the product. If separate files are used, maintain an attachment register so both parties can establish what was included with the order. A bare statement such as “same as sample” is difficult to use when several sample versions exist.

8. Connect branding and product identifiers to each variant

Avoid embedding a low-resolution logo in the PO and treating it as a substitute for the agreed production artwork. If one buyer supplies multiple brands, map each brand to the relevant order line and packing instructions.

Keep the buyer’s internal SKU, supplier style code and barcode identifier in separate fields; similar-looking codes may serve different purposes and should not be substituted automatically.

9. Specify packing and receiving requirements

Packing instructions should define the selling unit, folding or bagging requirements, size assortment within packs, carton labels and any agreed carton limits. Identify whether garments are packed by single size, mixed size or a nominated allocation. Connect every carton label to the order reference and product data so the receiving team can identify its contents before opening it.

10. Define the delivery schedule and approval dependencies

State the requested delivery schedule with clear milestones and identify which dates have been confirmed. Separate factory readiness, shipment and warehouse receipt. If the order is split into deliveries, allocate quantities to each shipment and confirm how partial quantities affect packing, documentation and receiving. A single final date is not enough when different products or colours are intended to arrive at different times.

11. Obtain supplier acknowledgement and control revisions

Ask for supplier acknowledgement that identifies the accepted PO version and any exceptions. A reply confirming receipt is different from accepting every quantity, specification and requested date. Resolve differences in writing and make the accepted reference clear to the people carrying out the work. Do not leave contradictory comments in separate messages with no consolidated decision.

Use revision control whenever an accepted field changes. Issue a new version, list the changes and identify what it supersedes. 12. Complete the final handover and receiving reconciliation

Before release, compare the PO total with the size matrix, price calculation, attachment register and delivery allocations.
